Psychological Evaluation Rates
Psychological Evaluations are needed for various reasons. Mindful Amity only administers these evaluations to children and adolescents. For adult evaluations, a referral can be made. All evaluations are paid in full on the first appointment. Fees include Intake/Initial Evaluation and Parent Feedback.

$850.00 Psycho-Educational (Includes IQ and Achievement Tests)
$550.00 Psycho-Educational (Includes IQ Test Only)
